The program needs people within health care organizations and community settings to act as local “champions” to help build awareness and support for the “Chronic Disease Self-Management” workshop. Could this be you?

Upon request, we will send posters, workshop flyers, PowerPoint presentations, and supporting resources to help you in your role as a self-management champion within your organization.

 The criteria for becoming a Faculty Member you must have a clinical background, willing to attend and successfully complete the training.
